A 18-year-old man presented to the surgical unit with acute pancreatitis On examination, he had eruptive xanthomata on arms and thighs . .

Plasma fasting lipids as the follows:

Sample grossly lipaemic.

Cholesterol 35 mmol/L (3.5–5.0)
Triglyceride 71 mmol/L (0.3–1.5)
HDL cholesterol 0.8 mmol/L (1.0–1.8)
Amylase 18 U/L (<200) .

1. ماهو الفحص الجيني الذي تثبت به الحاله؟
Lipoprotein lipase or APO C2

2. لماذا كانت نتيجه amylase منخفضة رغم ان المريض عنده
Acute pancreatitis?
due to the gross lipemic giving low serum amylase.

وكيف ستثبت ارتفاعه؟
Urinary amylase by spot urine sample measured to confirm .
3. اذا قسنا sodium ماذا تتوقع تكون نتيجته؟
Low due to the gross lipemic sample result in pseudohyponatraemia
4. حول قيم الدهون من mmol/L إلى mg/dl ؟
Conversion factors:
●Cholesterol, HDL and LDL → 0.0259
●TG → 0.0113

TG = 71 ÷ 0.0113 = 6283 mg/dl.
TC = 35 ÷ 0.0259= 1351 mg/dl.
HDL = 0.8 ÷ 0.0259= 30.9 mg/dl.
