A 54 years-old woman with a past history of systemic lupus erythematous, presented to our center with acute dyspnea.
Pulmonary CT angiography revealed bilateral pulmonary embolism. Due to high-risk features, the patient scheduled for catheter-directed thrombolysis; right ventricular perforation occurred during the procedure.
