clinical application of GUIDION by Dr Khosravani. Zanjan Mousavi Hospital.
The patient was an 82 year old diabetic man with a history of primary PCI on Ramus a week before this admition for staged PCI on RCA.
RCA was extremely calcified, therefore I used Grandslam wire (ASAHI INTECC). After stenting of the PDA and the distal part of RCA, I did predilation with NC 3 balloon on the proximal and midpart of RCA. I thought the midpart lesion is adequately predilated but I was wrong. After the stent deployment (Biomime 3.5 - 29), it had not been expanded at the midpart and looked dumbbel shaped and even more terrifying the stent balloon could not be extracted and I used much effort and force to do that. Then I used NC balloon, 3.5, 3 and even 2.5 (8mm length) to expand the stent but nothing could be passed through. Finally I used the Guidion (IMDS) and put it's tip inside the malexpanded stent and even passed through by inchworming technique which then I was succeded for balloon post dilation (by NC balloon 3.5)
