لو مريض زرع كبد وبعدها حصله HCV Recurrance
Recurrence of HCV infection is universal in patients with detectable HCV RNA at the time of liver transplantation
لازم بهلحالة نبدأ ب Anti viral ttt بسرعه ، غالبا بعد 3 شهور من ال Transplantation هيكون صار Stabilized ويقدر يأخد DAAs

لو كان مش Cirrhosis او Compensated cirrhosis
فعندنا خيارين للعلاج :
sofosbuvir and velpatasvir for 12 weeks
بدون Ribavirin
ميزة ال sofo/velpa انه ما بيتداخل مع ال Immunosuppresion drug اللي المريض هيكون ماشي عليها بعد ال Transplantation

glecaprevir and pibrentasvir for 12 weeks
ال Gleca/Pibernta بنحتاج نعدل جرعه ال Immunosuppresant ونتابع ال level تبعهم زي ال
Prednisone , Cyclosporin

لو المريض وصل لل DeCompensated Cirrhosis بعد ال transplantation
هياخد ال Sifo/Velpa مع اضافه ال Ribavirin لمده 12 شهر
sofosbuvir and velpatasvir with daily weight-based ribavirin(1,000or1,200mg in patients <75 kg or >=75kg,respectively)for 12 week

جرعه ال Ribavirin بنبدأ ب 600mg وبنزود حسب ال Tolerance

لو المريض كان Contraindicated لل Ribavirin
فبهلحاله بيمشي ع Sofo/Velpa لمده 24 اسبوع
