2022-04-11 07:34:24 Transesophageal echocardiography (TEE) should be performed prior to cardioversion to evaluate for atrial thrombus secondary to AF and heart structure and minimize risk for stroke. TEE is helpful for making the following determinations:
• To evaluate for atrial thrombus (particularly in the left atrial appendage)
• To guide cardioversion (if thrombus is seen, cardioversion should be delayed)
• When TEE is planned, the concurrent use of transthoracic echocardiography may increase cost without providing significant additional information

Fibrillatory waves may be evident or may be absent in patients with AF. QRS duration appears to be an independent predictor of incident AF among women but not in men on the basis of findings from 15,314 participants from the Atherosclerosis Risk in Communities (ARIC) study. The underlying mechanism for the difference between men and women is not yet clear.

Chest radiography findings are usually normal in patients with AF.

المحمول او الي يكون ضمن جهاز المونتر من الادوات الضرورية لكل طبيب كون Spo2 يعتبر أحد Vital signs

مهم ان نوضع الprobe بشكل صحيح
مو نعصر الاصبع (لان ممكن يوقف circulation ) ولا لكش losse حتى ميوگع او بقية الاضاءة الخارجية تاثر على القراءة

عند استخدامه بالأطفال يوضع اما على الابهام او big toe
واكو ear probe يفيدنا للاطفال

من نشوف الجهاز ميقره نفكر ان الاشكال بالجهاز فنجربه على standard الي هو احنه ونشوفه شغال لو لا

بعد ذلك نفكر بأسباب متعلقه بالمريض
منها
1. صبغ الأظافر لان ممكن الصبغة تمتص ضوء الاوكسيميتر
2. الحركة وطبعا هاي من اكثر الأسباب الي تقود الى قراءة خاطئة لذا مهم نثبت ايد المريض
3. Perfusion نقيس نسبة تشبع الدم بالاوكسجين فأذا ماكو دم بالاصبع فأكيد الجهاز مراح يقره صح ، ومن الاسباب الي تأثر على سريان الدم hypovolemia ، vasoconstriction from cold لذا لازم ندفي اليد قبل القراءة
4. CO poisoning راح تجينا قراءة طبيعية لان pulse oximetry is unable to distinguish between oxyhemoglobin and COHb. باز کردن / نظر دهید
