Testicular epidermoid cysts are the most common type of benign testicular tumors, and typically present in mid-adulthood (2nd to 4th decades).

Patients typically present with a painless non-tender testicular mass between 1-3 cm in diameter.

Ultrasound features demonstrate non-vascular, well-marginated intratesticular mass with a characteristic lamellated "onion skin" appearance (alternating hyperechoic and hypoechoic rings), hypovascular pattern by color Doppler.

MRI shows alternating rings of high and low signal at both T1 and T2 with no evidence of post contrast enhancement.
