Interesting catch. Patient with MI and large ventricular aneurysm, having multiple heart failure admissions and an 18 percent EF. We elected to perform ventricular remodeling surgery at Saint Alphonsus in Boise, ID. We resected the large ventricular aneurysm which I have marked in purple. You can see the clot. We then performed cryoablation with an atricure device to prevent malignant ventricular arrythmiae. We then used felt to buttress the closure. Finally, we came off and the EF had improved to 45 percent. We placed an IABP.
