Hypertensive Cerebellar Hemorrhage is an immediate life threatening condition that can lead to imminent death. This is a case of a young patient who became obtunded and fell into an immediate coma requiring a breathing tube with a 4cm cerebellar hemorrhage with ventricular hemorrhage and obstructive hydrocephalus and blood pressure in the 270s. Patient needed an immediate EVD followed by evacuation of the hemorrhage as shown bellow.
