مبدأيا هو ايه المشكلة لو حصل I.V injection ؟
المشكلة بكل بساطة أنه دا واحد من أشهر أسباب toxicity of local anesthesia
Injection of even a small amount of the anesthetic solution directly into the blood vessel can result in an immediate toxic level .
So it is critical to do aspiration each time an anesthetic is administered in a highly vascular area.
طيب استخدم ايه عشان اعمل aspiration ؟
1-self aspirating syringe

2-Harpoon type aspirating syringe
ودي متوفرة عندنا كلنا
To aspirate with the harpoon type syringe, you must be able to pull the thumb ring back several millimeters (1-2 mm)
You load the carpule , engage the harpoon adequately in the plunger and be sure that it is adequately engaged then insert the needle.
Disengagement of the harpoon from the plunger during aspiration occurs if the harpoon is dull or if you apply too much pressure to the thumb ring during aspiration. All What is needed is a gentle backward motion of the plunger, no need for forceful action.
Adequate aspiration also requires the tip of the needle remain unmoved and not pushed into the tissues or pulled out of the tissues during the aspiration test.

معناه أنه فيه arterial penetration
في الحالة دي change the cartridge and repeat the procedure.
يؤسفني اني أقول إنه كل كتب local anesthesia بتقول
You should aspirate multiple times during slow injection as negative aspiration doesn't guarantee that the bevel of the needle is not in the vessel
بمعني أنه ال needle tip ممكن تبقي داخل blood vessel لكن
bevel abuts the wall of the blood vessel
فلما تيجي تعمل aspirationهيبفي ال wall of the vessel sucked within the tip ويديلك false .
