كل المرضى اللي بيدخلو ب UGIB لازم ولا بد ياخدو PPI ..
اول شي بياخد المريض High Dose of PPI بيبدأ المريض ب 80mg من ال Esomeprazol ك I.V bolus

وبعدها بيكمل المريض ب (continuously Infusion (8mg/hr
وبينعمل للمريض Endoscopy

وحسب ال Endoscopy اما بيكمل المريض ع Infusion او بيكمل ع Orally ....

Early endoscopy — Our approach is to perform upper endoscopy within 24 hours for most patients with upper GI bleeding, but only after adequate resuscitation has been provided. For patients with suspected variceal bleeding, we perform endoscopy within 12 hours of presentation...

من خلال ال Endoscopy ال Ulcer بتنقسم ل Classes :

The appearance of ulcers can be described using the Forrest classification :

●Class Ia – Spurting hemorrhage

●Class Ib – Oozing hemorrhage

●Class IIa – Nonbleeding visible vessel

●Class IIb – Adherent clot

●Class IIc – Flat pigmented spot

●Class III – Clean ulcer base

لو كان المريض class Ia ,class Ib ,,Class IIa , Class IIb ...
هدول المرضى بيستمرو ع Continuous Infusion من ال PPI لمدة تلات ايام
يبقى في حالات ال Active bleeding او في Clot في ال Ulcer
او في Congested Vessel واحتمال تفرقع ..
فهدول الحالات المريض حيكمل ع PPI Infusion لمده 72 ساعه

اما Calss IIc ، Class III ،وجود Clear Ulcer بدون bleeding او Clot ف بيتشفت المريض ع Orally PPI

Such patients may be switched to a standard-dose oral PPI (eg, omeprazole 20 mg daily)

المريض اللي حيمشي على Orall PPI اما حيمشي ع
Standard dose of PPI as Omeprazol 20 mg daily

او حيمشي ع High Dose PPI
As omeprazol 40mg Twice Daily
كيف بنحدد هل المريض حيمشي ع Low or High dose PPI
في Score اسمه Rockall score ( مرفق بالصور)

A score of 2 or less is associated with a low risk of further bleeding or death.
المريض اللي Low Risk بيمشي ع Standard dose of PPI

اما لو كان ال Score اكتر من 2 , المريض رح يمشي ع High dose PPI لمده اسبوعين وبعدها بيتشفت ع Standard Dose

High-dose oral PPIs in patients with high-risk stigmata of recent hemorrhage are continued for a total of two weeks and then changed to a once daily dose
