تفسیر نوار قلب این بیمار


تریاد هیپرکالمی + هیپوکلسمی + فشار خون (LVH) قویا به نفع رنال فیلر هست.


1. سینوس برادی کاردی با ریت 41bpm
2. انحراف محور قلب به سمت راست (°98)
3. هایپرکالمی، هیپوکلسمی
4. هایپرتروفی بطن چپ (LVH)


Ravl + Sv3 > 20 women 28men

با توجه به Cornell Criteria، وجود S عمیق و بیش از 28 مربع کوچک ! در لید V3 بیمار کیس LVH هست

از طرفی دیگر با توجه به QT برابر با 600ms با یک QT Prolong مواجه میباشیم که برای محاسبه Qtc از فرمول های زیر استفاده میکنیم

• Bazett Formula: QTc = QT interval / √ (RR interval)
• Fridericia Formula: QTc = QT interval / (RR interval)1/3
• Framingham Formula: QTc = QT interval + 154 x (1 - RR interval)
• Hodges Formula: QTc = QT interval + 1.75 x [(60 / RR interval) − 60]

- RR interval = 60 / HR

فرمول Hodges مثبت کاذب کمتر دارد، QT بیمار 15 مربع کوچک میباشد که برابر 600 میلی سکند است و HR بیمار که 41 بیت پرمینت است

QTc = (600ms + 1.75) × (41-60) = 576ms

طبق رفرنسهای AHA & ACC میزان حداقل و حداکثر نرمال QTc درخانم ها و آقایان بشرح زیر است : حداکثر نرمال برای خانم ها 460ms برای آقایان 450ms و حداقل نرمال در هر دو جنس 390ms

زیباترین وصف برای هایپرکالمیا :

The T waves associated with hyperkalemia are often referred to by a multitude of descriptors such as: tall, symmetrical, narrow-based, “thorn-like”, peaked, sharp, pointed, tented, and prominent. 

Some even refer to them as tombstone or hyperacute but you run the risk of confusing others because both of these are also used by certain authors to describe T-waves in the presence of acute myocardial infarction.  Of various descriptors, I probably like the 2nd of “Taigman’s T-wave Rules” the most.

Rule 2: “If the T-waves look like they would hurt your butt if you sat on them, the patient is probably hyperkalemic.” (1)  Most of those precordial T-waves do indeed look like they would be very painful to perch your hindquarters upon.
