من الحالات التي تحدث عند استعمال #البربوفول هي متلازمة البربوفول التسريبي #propofol_infusion_Syndrome ....

• عند اعطاء المريض Propofol مدة تتجاوز 48 ساعة وبجرعة تسريبية infusion عالية بمقدار اكثر من 5 ملغم/كغم/ساعة .

• يصاب المريض بالاتي:
- تأيض حامضي شديد metabolic acidosis.
- ارتفاع البوتاسيوم بالدم .
- حدوث Lipemia تعني تجمع ال lipid بالدم .
- حدوث Rhabdomyolysis وتعني
a serious syndrome due to a direct or indirect muscle injury. It results from the death of muscle fibers and release of their contents into the bloodstream.
- حدوث تضخم بالكبد او تجمع الدهون عليه hepatomegaly وقد يحدث فشل بالقلب و الكلى .

• الاعمار الشابة والاطفال هم اكثر عرضه لهذه الظاهرة او المرضى الذين يأخذون cortisone infusion .

#العلاج
1- high index of suspicion
2- monitor for early warning signs (lactate, CK, urinary myoglobin, ECG)
3- discontinue propofol immediately
4- supportive care and monitoring
5- adequate carbohydrate intake (6-8mg/kg/min)
6- carnitine supplementation (theoretical benefit)
7- haemodialysis and haemoperfusion (used with success in case reports)
8- ECMO (at least 2 cases reported, reasonable strategy given readily reversible pathology)
