Massive paraganglioma (glomus jugulare) mass of left ear in a 50 years old woman presented with chief complaint of persistent pulsatile tinnitus and hearing loss of left ear and hypoglossal nerve paresis on examination (Red arrow showing deviation of tongue toward the mass). On axial temporal bone HRCT an erosive skullbase mass causing the jugular foramen irregular widening (blue arrow), dehiscent posterior canal wall of carotid canal (white arrow) and extension of the mass to the external auditory canal (yellow arrow) were seen causing an otoscopic view of red fleshy mass, completely occluding EAC and covering tympanic membrane.

ICE in lead extraction!
Not being able to pass TEE probe easily, I then used ICE during a lead extraction procedure.
I think ICE provides better n more imaging for lead extraction with more control in hands of primary operator !

Update:
Unfortunately despite all efforts this patient passed away in the ICU. As time went on we learned the patient had progressively worsening COPD and DM. He did required CRRT.

81yo/m presents to the ED complaining of difficulty breathing over the last several days. He denies being around anyone positive or high risk for COVID-19. He further denies any chest pain along with N&V. Initially his SpO2 is 75% on RA. During your assessment you note +2 edema in the peripheral lower extremities. The patient denies any history of CHF. Shortly after the patient becomes unresponsive with a 10 second period of asystole on the monitor. Quickly regains consciousness with a rhythm of sinus brady. Below is the ekg. Now it’s your turn. What would like to know and what is our course of action?
