82-year-old patient with a history of bone tuberculosis and recurrent Staphylococcus aureus infection in the right hip with several reoperations for debridement. Several years of chronic pain in the spine that conditions functional impotence for my ambulation, which is why she attended. An X-ray is performed which shows: severe left convex lumbar scoliosis. Marked degenerative changes, with anterior and posterior marginal osteophytes, associated with diffuse bulging of the discs, more accentuated in L3-L4, L4-L5 and L5 S1. In L5-S1, a left foraminal hernia is also identified. Severe hypertrophy of the facet joints, which, together with the findings previously described, conditions a segmental stenosis of the canal at L4-L5. Significant Meyerding grade 3 lumbar listhesis in conjunction with L5-S1 spondylolysis. Medullary cone without apparent alterations. باز کردن / نظر دهید

2021-05-09 07:38:28 Bronchopleural fistula (BPF) is associated with high morbidity if left untreated. Although rare, the frequency of BPF in severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is becoming recognized in medical literature. We present a case of a 64-year-old male with BPF with persistent air leak due to SARS-CoV-2 pneumonia treated with Spiration Valve System endobronchial valve (EBV). An EBV was placed in the right middle lobe with successful cessation of air leak. In conclusion, the use of EBVs for BPF with persistent air leaks in SARS-CoV-2 patients who are poor surgical candidates is effective and safe. باز کردن / نظر دهید

Last weekend she went to a party, had fun, drank alcohol and for the first time in her life she tried smoking tobacco. She woke up on Monday morning with these changes in her mouth.
Patient says she had no problems before the weekend.

A constant burning sensation.

No changes otherwise in the mouth

Factitious Disorder Masquerading as a

2021-05-02 11:58:24 Factitious disorder is a psychiatric disorder in which sufferers intentionally fabricate physical or psychological symptoms in order to assume the role of the patient, without any obvious gain. We present a case of a 23-year-old female with chronic urticaria who presented with dyspnea, dysphasia, mild generalized erythema, abdominal cramps, and headache. She was tachypneic and hypotensive. This was her third admission with similar symptoms within the last 7 months. Tryptase, complement, anti-SM/RNP, Sjogren, Scl-70, C3, and C4 were negative. Computed tomography–guided bone marrow biopsy showed no mast cells. Flow cytometry did not show any immunophenotypic reaction. Other possible differentials including pregnancy, autoimmune disorders, and infections including hepatitis, thyroid disorder, and age-related malignancies were ruled out. After a thorough review, malingering disorder was ruled out, but we noticed the patient’s intent of assuming a sick role. باز کردن / نظر دهید
