Verruca vulgaris! is a benign lesion mimicking SCC (squamous cell carcinoma) usually seen on skin and then mucous membranes caused by human papillomovirus (HPV) especially types 1, 2, 27 and 57. The lesions are typically self-limited and occurrence on the tongue is extremely rare. Here is a case of 41 years old woman with giant 2*4 cm tongue verruca vulgaris lesion from 6 months ago (orange arrow). Pathology report of biopsy showed no evidence of malignancy and T1 MRI detects no sign of submucosal invasion or malignancy suspected signal as seen below (green arrow). Liquid nitrogen cryotherapy is the treatment of choice and PDL (pulsed dye laser) is used for recalcitrant lesions.

A 50-year-old man with chest pain and shock is admitted in emergency room and the ECG is recorded.
In the ECG you see baseline artifact (up and down baseline), which is common in critically ill patients,but despite this, interpretation is possible. Sinus tachycardia is present at a rate of 100 beats/min. The axis and intervals are normal, and there is no evidence of chamber enlargement or hypertrophy. There are ST-segment elevations in the inferior leads II,III, and aVF. There are only tiny, nonpathologic Q waves present in those leads with upright T waves. Significant ST-segment depression is present in leads I and aVL as well as leads V1 through V3.
The ST-segment elevations in leads II, III, and aVF correspond to ischemia of the inferior wall of the left ventricle. Inferior wall ischemia is typically due to occlusion of either the right coronary artery or the left circumflex coronary artery.

Sinus rhythm with left atrial abnormality. Most striking are tall right precordial R waves with pathologic inferior and lateral Q waves giving a "reverse" type R wave progression. There are also ST-T abnormalities in I, aVL, V6. Thus the patient has coronary artery disease (CAD), s/p large infero-(postero)-lateral myocardial infarction (MI) accounting for congestive heart failure (CHF) findings. Recall that differential diagnosis of tall right precordial R waves includes: 1) Normal/positional variants, 2) Right ventricular hypertrophy (RVH) (look for right axis deviation, P pulmonale), 3) Posterior/lateral MI (usually signs of inferior MI, too), 4) hypertrophic cardiomyopathy/idiopathic hypertrophic subaortic stenosis (HCM/IHSS), 5) right bundle branch block (RBBB), 6) Wolff-Parkinson-White (WPW ) variants with posterior/lateral pre-excitation 7) Duchenne muscular dystrophy (young men with myopathy).

This patient has a very large lumbar disc herniation. This patient does not need surgery. He is much improved after a course of anti-inflammatory medications and physical therapy. Unless there are worrisome symptoms, patients, and certainly surgeons, should not rush into the surgical option.

62 year old female, non-responsive to conservative care with new symptom of "pulling sensation" down the legs - this is probably a meningioma (yellow arrow).
Just a reminder to look again for the cause of patient's issues when your initial plan isn't quite working, and that XR can commonly miss very relevant findings.

In women who have (Recurrent Spontaneous Abortion = RSA) Or (Habitual abortion), the genetic formula (karyotype) of the parents before re-pregnancy is very important. Abortion may occur for genetic reasons.
Of course, there may be different reasons, but because of the cost of karyotype testing, parents refuse to do so!
