This is a lateral chest X-ray of one of my patients, a 53 Y/O male a case of Takayasu arteritis, it is very interesting because all aortic contour is visible due to heavy calcification of the aortic wall.

A 53 years old man with an incidental finding of a cholesteatoma of left ear! He was complaining of right sided hearing loss with a history of previous tympanoplasty and during otoscopy we detected a cholesteatoma pearl and superoposterior perforation of the left tympanic membrane (blue arrow).
The most interesting part is while the diagnosis of cholesteatoma was almost definetly known according to the otoscopy view and temporal bone CT scan showing middle ear soft tissue opacity with bony boundaries erosions (red arrow), the PTA and tympanometry showed normal values! Even the patient himself said my left ear hearing is better than right side! He was right! This is called mass effect!
This point indicates the importance of patient orientation to her hearing status after mastoidectomy and tympanoplasty in the setting of cholesteatoma.

Fibromuscular disease of Carotid arteries in 61 y.o. Lady with history of head aches , hypertension and type 2 diabetes. She had normal coronary arteries. Bilateral carotid FMD changes with severe LICA involvement and subtotal occlusion of para ophthalmic ( C6) segment of left internal carotid artery.

Hypertensive Cerebellar Hemorrhage is an immediate life threatening condition that can lead to imminent death. This is a case of a young patient who became obtunded and fell into an immediate coma requiring a breathing tube with a 4cm cerebellar hemorrhage with ventricular hemorrhage and obstructive hydrocephalus and blood pressure in the 270s. Patient needed an immediate EVD followed by evacuation of the hemorrhage as shown bellow.

Not all “scoliosis” is true scoliosis. When patients complain of sudden back and leg pain followed by twisting of the back, a disc herniation diagnosis has to be considered and treated first.

Postural (or reactive) scoliosis due to pain will likely resolve and not need major corrective surgery.

Yet another example why taking the time to listen to the patient (and not just focus on x-rays) is so important.

I am sharing with you 1-year follow-up X-rays on a 42 yo active duty military gentleman with a 3-level cADR. Previously, in September of 2021, I shared on this platform a brief video at his 3 mo follow-up visit with this same patient stating how happy he was with the results. In this patient's case, he continues to report pain-free full ROM 1 year from the time of surgery.

Frequently, I see significant sagittal correction right after arthroplasty surgery as the anterior column height is restored and motion segments are mobilized. With longer follow-ups, I always like to look at serial x-rays to assess any changes, be they positive or negative. As I arranged his preop x-ray on the right and the subsequent follow-ups at 3 weeks, 3 months, and now 1 year to the left, I was struck by the beautiful progressive restoration in his sagittal alignment.

I am wondering if the surgeons within our LinkedIn community are observing similar findings in their patients.

Interesting catch. Patient with MI and large ventricular aneurysm, having multiple heart failure admissions and an 18 percent EF. We elected to perform ventricular remodeling surgery at Saint Alphonsus in Boise, ID. We resected the large ventricular aneurysm which I have marked in purple. You can see the clot. We then performed cryoablation with an atricure device to prevent malignant ventricular arrythmiae. We then used felt to buttress the closure. Finally, we came off and the EF had improved to 45 percent. We placed an IABP.

In patients with anemia, the AGA recommends using a cutoff of 45 ng/mL over 15 ng/mL when using ferritin to diagnose iron deficiency. Strong recommendation,high-quality evidence.

Iron deficiency is very common, especially among women and in people who have a diet that is low in iron. The following groups of people are at highest risk for iron-deficiency anemia:

●Women who menstruate, particularly if menstrual periods are heavy
●Women who are pregnant or breastfeeding or those who have recently given birth
●People who have undergone major surgery or physical trauma
●People with gastrointestinal diseases such as celiac disease (sprue), inflammatory bowel diseases such as ulcerative colitis, or Crohn disease
●People with peptic ulcer disease
●People who have undergone bariatric procedures, especially gastric bypass operations
●Vegetarians, vegans, and other people whose diets do not include iron-rich foods (Iron from vegetables, even those that are iron-rich, is not absorbed as well as iron from meat, poultry, and fish.)
●Children who drink more than 16 to 24 ounces a day of cow's milk (Cow's milk not only contains little iron, but it can also decrease absorption of iron and irritate the intestinal lining causing chronic blood loss.)

Gastrointestinal evaluation of iron deficiency anemia

Iron is very important in maintaining many body functions, including the production of hemoglobin, the molecule in your blood that carries oxygen. Iron is also necessary to maintain healthy cells, skin, hair, and nails.

Iron from the food you eat is absorbed into the body by the cells that line the gastrointestinal tract; the body only absorbs a small fraction of the iron you ingest. The iron is then released into the blood stream, where a protein called transferrin attaches to it and delivers the iron to the liver. Iron is stored in the liver as ferritin and released as needed to make new red blood cells in the bone marrow. When red blood cells are no longer able to function (after about 120 days in circulation), they are re-absorbed by the spleen. Iron from these old cells can also be recycled by the body.
