عرفنا بالبوستات السابقة انه ال IHD هنقسمها لنوعين كبار
اول نوع هو ال Chronic Coronary Syndrome(CCS)
والنوع التاني هو ال Acute Coronary Syndrome (ACS)

وشفنا ال Causes اللي بتدخل المريض ب IHD
وكان اشهر سبب هو ال Atherosclerosis
ف 90% من حالات ال IHD سببها ال atherosclerosis

وبنقسم ال IHD لأربع حالات على حسب قديش ال atherosclerosis Plaque هتضيق الCoronary Lumen

less than 50%
لو كان الانسداد اقل من 50% فبيكون Asymptomatic والمريض مبيشتكيش خالص
50-70% Obstruction
بيكون الدم الواصل لعضله القلب كافي At Rest
لكن بمجرد بذل أي مجهود بيحصل Chest Pain
وهاد بنشوفه في حالات ال CCS
70-99% Obstruction
بيحصل ال Chest pain حتى خلال ال Rest

Obstruction of Lumen 100% Acute MI ( STEMI)


كيف بتتكون ال atherosclerosis Plaque داخل ال Coronary

ال LDL( الكوليستيرول السئ) بيصيرله Oxidation وبيلزق داخل ال Aretry خاصة لو في Endothelium Dysfunction او Inflammation

Is an inflammatory fibro-proliferative process Affecting the intimal layer of arteries Leading to accumulation of cells, lipids, fibrous tissue, and calcium → Resulting in the formation of an atherosclerotic plaque → Ending with progressive luminal stenosis ( Source : Cardiology secret Code)

طيب شو هي Risk factor لل Atherosclerosis ?

Modifiable Risk Factor
هاي ال Risk factor اللي بنقدر نعمللها Management
زي ال HTN وال DM وال Smoking وال Stress وال obesity
وال High LDL level

Non-Modifiable Risk Factor
عوامل ما بنقدر نعدلها

عشان يصير Ischemia بال Cardiac tissue بيكون صار في Imbalance بين ال Blood supply of heart وبين ال Heart Demand
Decreased in Cardiac Blood Supply

القلب واصله دم أقل من المطلوب فالخلل في ال Supply

Increas in Cardiac tissue Demand
هنا القلب واصله دم بكمية كافيه لكن احتياجات القلب زادت..



شو هي الأسباب اللي ممكن تقلل ال Cardiac blood Supply

Low Cardiac output...

لو كان القلب بيطلع CO قليل فحصة القلب من ال CO هتتأثر ويوصله أقل من المطلوب زي حالات ال Heart Failure او ال Pulmonary embolism او ال Aortic stenosis

Coronary Artery Disease ( CAD)

ال Coronary Aretry بأفرعه هو المسئول عن ال blood supply للقلب
فأي مشكله في ال Coronary رح تأثر على كمية الدم الواصلة للقلب
مشاكل ال Coronoray بنقسمها ل 4 حاجات ..

لو كانت المشكله في فتحة ال Coronary

في حالات بيكون عندها congenital osteal stenosis ضيق في فتحة ال Coronary artery
او في حالات tertiary syphilis بيضيق فتحه ال Coronary
syphilitic osteal stenosis

لو كانت المشكله في ال Coronary lumen

لو صار thrombosis Or embolism بيضيق ال Lumen وبيعيق ال Blood flow
مثلا مريض AF ممكن يبعت Emboli لل Coronary

او مريض Hyper coaguable state زي حالات ال

anti phospholipid syndrome
Protein C and S deficiency
Sickle cell anaemia in young age

بيكونو عرضة لل Thrombosis

لو كانت المشكلة في ال coronary endothelium

زي حالات
ال Atherosclerosis او vasculitis

حالات ال Atherosclerosis بتشكل 90% من اسباب ال IHD

لو كانت المشكلة في ال Coronary wall
Corobary wall spasm Or dissecting aortic aneurysm



اما الأسباب اللي بتزود ال Cardiac Demand هي :

thyrotoxicosis
cardiac hypertrophy as in HTN ,Aortic stenosis
Hypertrophic cardimyopathy
